Billing Information for Health Plans
For any billing questions, please contact our billing professionals at 1-866-621-5234. Geisinger Proven Diagnostics® will bill your company directly using the standard 1500 form. We can also bill electronically or via a paper form if required. We can also bill you directly for preventive care services for your clients and bill patients for any copays or deductibles.
Geisinger Proven Diagnostics requires a valid diagnosis code for each laboratory test we perform.
Our billing staff will handle rejections, edits and diagnosis claim issues for your clients.
If you are not a participating health plan, a preauthorization is required in order for us to perform and bill for services.
