Proven Diagnostics, the name of our laboratory This particular test is a panel, a group of tests performed together. These are the tests that make up the basic metabolic panel. The name of the test that was performed The patient's date of birth An identification number assigned to the specimen The patient's age and sex. Some test results vary based on age and sex of patient, so it is important that we have this information. The address from which the test was ordered An identification number assigned to the patient The patient's phone number The name of the patient on whom the test was performed Information about who is to be billed for the test The results for each test Abnormal results are those outside the reference range. Here, the patient has a glucose of 250, which is higher than the reference range of 70-120 mg/dL. The reference range is considered the range of normal values for the test The date and time the test was received at the lab. The name of the physician who ordered the test and to whom results will be reported The name of the laboratory in which the test was performed An identification number assigned to the patient. The date and time the specimen was collected. The date and time the test was resulted The status of the test. F means it is final.
